Vanc App 'Vancomycin First-dose Trough Dose Adjustment Calculator for Young Infants'
is a dose adjustment calculator for young infants receiving intermittent vancomycin.
It recommends if a dose adjustment is required to achieve an AUC24-48 of 400-650 mg/L·h (24-48 hours after commencing vancomycin) based on an individual’s post-menstrual age,
weight, serum creatinine level, dose, dosing interval and first-dose trough level
(taken immediately prior to the time the second dose is due). If the steady state trough is < 5 mg/L or > 20 mg/L, dose interval will be adjusted.
Vanc App calculators are intended for use in young infants aged 0 to 90 days with a post-menstrual age ≥ 25 weeks and weight ≥ 500 grams.
They have not been validated in young infants with abnormal renal function, those receiving extracorporeal membrane oxygenation or renal replacement therapy.
